Out of hospital care undergoes a shake-up

PROVISION of out of hospital healthcare in Warwickshire has undergone a shake-up.

Warwickshire County Council and South Warwickshire Foundation Trust have integrated some of their services to create the countywide HomeFirst.

It will provide short term services to people if they have experienced an unexpected health crisis or a change in their health which has affected their abilities to do daily living tasks.

Spokeswoman for adult social care, Coun Jose Compton said: “The integration of the two teams will provide a more joined-up responsive single service across the county and will help to ensure a better experience for people who are at a time in their lives when they feel vulnerable.




“It will help to prevent some people being admitted to hospital or requiring 24 hour care while others who are in hospital will benefit from a timely discharge from hospital. There are real benefits for our users to be realised by us working in this way.”

Those wanting to find out more about the HomeFirst service can attend a free session at the Sydni Centre in Leamington on Monday February 27, from 2pm.
